9 - Community-Based Pharmacy – Case Study Care Optimization through Application of Technology Innovation

This session will provide a window into APhA Foundation’s efforts to combine health technology innovations with optimized processes of patient-centered, team-based care that includes pharmacists as a compelling solution for improving our healthcare delivery system. The Solutions for Atrial Fibrillation Edvocacy (SAFE) initiative illustrates how hybridized modeling creativity with a new consumer-technology innovation through highly accessible community pharmacy practice has the potential to improve peoples’ health, lives, and population health. Participants will learn about this patient-centered, personalized care model that takes a best-in-class technology innovation and fuses it with the unique access, knowledge, and skills of humans who are passionate about improving lives – pharmacists!!
