Jason Martinez, PharmD, BCACP is the Chief Population Health Officer for Community Health & Wellness Partners where he developed the first clinical pharmacy position at his FQHC located in rural Ohio. He oversees CHWP’s multi-disciplinary Chronic Care Management program, where disease state management, an anticoagulation service, tobacco cessation, a pharmacogenomics clinic, and an accredited diabetes education program are delivered by pharmacists, nurses, dietitians, and social workers. As an Apexus Certified 340B Expert, he provides knowledge, oversight, and expansion of 340B pharmacy services helping patients access vital medications. Dr. Martinez was awarded the Ohio Pharmacists Association Under 40 Award in 2020. He received his Doctor of Pharmacy from Ohio Northern University and is Board Certified in Ambulatory Care Pharmacy.
